A hospital trust has been ordered to pay 100,000 after a new mother "needlessly" died when an epidural anaesthetic was mistakenly fed into her veins. Mayra Cabrera, who was 30 years old, died of a heart attack one hour after giving birth to a healthy baby, Zachary, at the Great Western hospital in Swindon, where
